Capturing the sixth vital sign – where life happens

The Axis Patch is currently intended for research use only.

The Grasshopper Axis Patch™

The Axis Patch™ is a lightweight, discreet wearable designed to capture continuous movement data where patients actually live, move and recover, not just in the clinic.

Placed comfortably at the sacrum, the body’s biomechanical fulcrum, the patch continuously detects subtle gait patterns invisible to the human eye. This unique positioning enables high-fidelity, real-world data collection without requiring behavior change, active engagement or technical setup from the patient.

No wires, burden or disruption to daily life. Just continuous insight.

Grasshopper Axis Patch™ with Box and Quarter

From Real-World Movement to Clinical Action

The Axis Patch™ transforms everyday activity into objective, longitudinal health data, enabling care teams to monitor patients remotely and respond earlier when something changes. Key capabilities include:

clinical-icon1

Remote monitoring across the care continuum, from post-acute recovery to chronic disease management

clinical-icon2

Detection of subtle functional changes that may signal deterioration or lack of therapeutic response

clinical-icon3

AI-driven analysis trained on large-scale biomechanical datasets for clinical insights

clinical-icon4

Passive data capture with no patient input for continuous insight into real-world adherence and recovery patterns

Grasshopper Motus™ Platform

The Grasshopper Motus™ Platform

The Motus™ Platform is built to integrate seamlessly into existing provider workflows, delivering actionable insights without adding clicks, dashboards or administrative burden.

Rather than creating a new system to learn, Grasshopper works within the systems clinicians already use, such as Electronic Health Record platforms, ensuring that gait data becomes part of routine care delivery.

How It Works

card-img1
Step 1
Continuous Capture

The Axis Patch™ passively records gait and mobility patterns during daily life: at home, in the community and during recovery.

Phone Data Transfer
Step 2
Secure Transmission

Data is transmitted via the patient’s smartphone to the Motus™ Platform, enabling always-on remote monitoring without friction.

Walking AI-Powered Analysis
Step 3
AI-Powered Analysis

Advanced models identify deviations, trends and early warning signals across neurological, vascular and musculoskeletal conditions.

Provider with Laptop
Step 4
Clinical Integration

Insights are delivered directly into the provider’s workflow, embedded within the EHR, so clinicians can act without leaving their existing environment.

Provider with Patients
Step 5
Patient Engagement

Patients and family members receive simple guidance and timely progress updates to reinforce adherence and shared care accountability.

Features & Functionality

Designed for real-world care delivery

Clinician on Phone

Workflow Integration

Grasshopper fits naturally into clinical operations, so providers get clear, prioritized insights – not raw data – enabling faster, more confident decision-making.

Features include:

  • Ordered directly within the EHR, no new prescribing systems
  • Automated fulfillment and enrollment
  • Data flows back into the chart, not into a separate silo
  • Supports reimbursable remote monitoring and telehealth workflows

Patient Experience

Patients stay connected to their care team through subtle nudges and feedback loops, while caregivers can be included to support adherence and safety when needed.

Features include:

  • Device shipped directly to the home
  • Simple onboarding via telehealth
  • Passive operation with no daily tasks required
  • Smartphone connectivity enables real-time communication

Disease States

Gait is one of the earliest indicators of health decline across multiple conditions, often changing months or years before symptoms are reported. By continuously measuring speed, symmetry, variability and stride dynamics, Grasshopper provides earlier insights and ongoing monitoring of various health conditions.

Disease states include:

  • Peripheral Artery Disease (PAD)
  • Parkinson’s Disease
  • Normal Pressure Hydrocephalus (NPH)
  • Alzheimer’s Disease

Remote Monitoring

By capturing data in the home and integrating it into clinical workflows, Grasshopper helps close the gap between what clinicians prescribe and what patients actually experience.

Features include:

  • Remote Patient Monitoring (RPM)
  • Longitudinal functional tracking
  • Early intervention triggers
  • Population-level insights for health systems
